Please complete this form for each participating volunteer. Each form is valid for 12 months.
Release of Liability
By selecting "yes", you hereby release The Bull City Woodshop (“BCW”) its staff, board and volunteers from any and all liability for damages for property loss and/or injuries sustained by you while present on the BCW premises at 1912 E Pettigrew St, Durham, NC or while participating in any BCW program activities off the premises, whether as a volunteer, program participant, parent/guardian, community worker, visitor or in any other capacity.
Safety Release
By selecting "yes", you certify that you do not suffer from any medical, physical, psychological or other condition that would impair your ability to safely operate power tools and fully participate in The Bull City Woodshop (BCW) woodworking and outreach programs. You also certify that any medications you take will not interfere with your ability to be alert and attentive during The BCW classes. Furthermore, all information you have given is up to date and correct at this time. You also agree to inform the staff of The BCW if and when any of this information changes to ensure proper care and safety. The BCWs staff constantly assesses the ability of all youth, participants and volunteers to work safely, follow instruction and meet the expectations of The BCW program.
Photo/Video Release
By selecting "yes", you give your permission for The BCW to use photographic and/or video images of yourself for program purposes, on The BCW literature, The BCW brochures, on The BCW website, on social media and in media stories about The BCW or in other lawful and appropriate ways related to The BCW program. You understand that such photographic and video images may be distributed to granting sources and others who may utilize the images in reporting about or promoting The BCW program.
Emergency Call Release
By selecting "yes", you authorize The BCW staff or volunteers to call emergency services in the case of an emergency.
Mandatory Reporting
By selecting "yes", I agree that I have been provided a copy of North Carolina’s Mandatory Reporting law listed below, and I understand that North Carolina has a universal mandatory law stating all forms of abuse/neglect need to be reported.
Drug Free Workplace

By selecting "yes", you understand that the BCW Volunteer Handbook contains proprietary information. This information is necessary for your role as a volunteer, and you are trusted to keep it confidential.

By selecting "yes", you agree to comply with the policies contained in this handbook, and any revisions made to it. You further agree that if you remain with the Bull City Woodshop following any modifications to the handbook, you thereby accept and agree to such changes.
North Carolina’s Universal Mandatory Reporting Law and Confidentiality
§ 7B-301. Duty to report abuse, neglect, dependency, or death due to maltreatment.
(a) Any person or institution who has cause to suspect that any juvenile is abused, neglected, or dependent, as defined by G.S. 7B-101, or has died as the result of maltreatment, shall report the case of that juvenile to the director of the department of social services in the county where the juvenile resides or is found. The report may be made orally, by telephone, or in writing. The report shall include information as is known to the person making it including the name and address of the juvenile; the name and address of the juvenile's parent, guardian, or caretaker; the age of the juvenile; the names and ages of other juveniles in the home; the present whereabouts of the juvenile if not at the home address; the nature and extent of any injury or condition resulting from abuse, neglect, or dependency; and any other information which the person making the report believes might be helpful in establishing the need for protective services or court intervention. If the report is made orally or by telephone, the person making the report shall give the person's name, address, and telephone number. Refusal of the person making the report to give a name shall not preclude the department's assessment of the alleged abuse, neglect, dependency, or death as a result of maltreatment.
(b) Any person or institution who knowingly or wantonly fails to report the case of a juvenile as required by subsection (a) of this section, or who knowingly or wantonly prevents another person from making a report as required by subsection (a) of this section, is guilty of a Class 1 misdemeanor.

Report any suspected child abuse without delay to Durham County Department of Social Services (DSS). To report suspicions of abuse or neglect to the Durham County DSS:
• Monday through Friday, 8:15am-5:30pm: Call (919) 560-8424
• After hours, weekends or holidays: call 911 and speak with a Child Protective Services Intake Social Worker
• You may also make a report in person at 220 E. Main Street, Durham NC.
After reporting suspected child maltreatment, staff and volunteers must:
• Inform their immediate supervisor as soon as possible. Staff and volunteers are under no obligation to give names or details to any parties other than law enforcement and DSS.
• Understand that it is not their responsibility to conduct an investigation. It is the responsibility of the Durham County DSS to substantiate all claims and suspicions of child abuse or neglect.
Everyone has a duty to report in cases where there is suspected child abuse and/or neglect by a parent, guardian, custodian or caretaker.
If any such allegations or suspected abuse/neglect is suspected the leadership team of The Bull City Woodshop (BCW) will be notified and will follow up with the proper action.
DRUG FREE WORKPLACE POLICY
The Bull City Woodshop is committed to providing a safe work environment and to promoting and protecting the health, safety, and wellbeing of our employees, volunteers, and students. This commitment is jeopardized when any Bull City Woodshop employee/s or volunteer/s engages in use, possession, sale, conveyance, distribution or manufacture of illegal drugs, intoxicants, controlled substances or abuses prescription drugs or alcohol. Substance abuse is a significant public health
problem, which has a detrimental effect on the business community in terms of productivity, absenteeism, accidents, medical costs, theft, and workers’ compensation costs. Therefore, The Bull City Woodshop has established the following policy:
1. It is a violation of company policy for any employee or volunteer to use, possess, sell, convey, distribute, or manufacture illegal drugs, intoxicants, or controlled substances, or to attempt to do the same.
2. It is a violation of company policy to use or be under the influence of alcohol anytime during hours of business operation while on or using company property.
3. It is a violation of company policy for anyone to use prescription drugs illegally. It is the responsibility of the employee/volunteer to report the use of prescribed drugs that MAY (per warning labels provided by the pharmacy) affect the employee’s judgment, performance, or behavior.
4. Violations of this policy are subject to disciplinary action up to and including termination of employment.
The Bull City Woodshop values its employees and volunteers and recognizes the need for a balanced approach to achieving a drug free workplace. Therefore, we are committed to conducting an ongoing drug free awareness program for our employees/volunteers and advising them of resources for drug counseling, rehabilitation and other assistance.
The Bull City Woodshop drug free workplace policy is intended to comply with all state and federal laws governing drug and alcohol testing and is designed to safeguard employee/volunteer privacy rights to the fullest extent of the law.
As a term and condition of employment with The Bull City Woodshop each employee/volunteer agrees to:
1. Abide by the terms of the above policy; and
2. Notify The Bull City Woodshop in writing of his or her conviction for a violation of a criminal drug statute occurring in the workplace no later than five calendar days after such conviction.
